medications at school

If a student requires medications to be administered at school:
1) The medication must be hand-delivered by an adult to the Nurse
2) A parent authorization form must be signed by the parent, completed and given to the Nurse
3) A provider order form must be signed by the doctor, completed and given to the Nurse

If your child needs a Prescription Medication administered at school, please complete the below form in full.
If your child requires the administration of an over-the-counter medication at school, please complete the below form.
If your child has an epi-pen please complete the form above for prescription medication and this form below.
If your child has an inhaler and you wish to have them carry it on themselves and self administer, please complete the form above for prescription medication and this form below.
